If you are able to, feel free to download the Initial Session Paperwork and bring it to your first session. To do so, click the link below, print it out, and fill in the responses. If you are not able to print out the paperwork and fill it in, please arrive to your initial session 30 minutes ahead of time so that you are able to fill it out in the waiting room. I will go over all paperwork with you during our initial session and will be happy to answer any questions you may have.
